Project Description:

 

 

 

 

 

 

 

This is a proposal to inter-connect DCF District 14 primary agencies, governmental services and providers, particularly children's services and substance abuse services, via an interactive Information System.  The goal is to create a seamless information and referral process which best utilizes existing community services, maximizes efficient service-delivery dollars, enhances family independence, decreases reliance on public funds, and allows for economies that can be re-invested in a tighter and more accessible continuity of care.

 

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

The main District 14 providers and governmental agencies will enjoy far more rapid and interactive communitcations on service availablity and outcomes, while children and families will gain wider accesss to multiple levels of community services from prevention, intervention, and support to rehabilitation and treatment alternatives.  The project is designed to maximize use of existing services and available monies to minimize duplications.  As 80% of District 14 mental health and substance abuse services are provided by the three agencies involved in this proposal, this should provide a strong infrastructure upon which to add other agencies in the future.

 

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

Completed internet & Information System interface between Winter Haven Hospital's Behavioral Health Division and Peace River Center, and beginning interface with Tri-County Human Services, Inc.  Implementation of interactive web-sites that can be accessed by any providers &/or community services for information & referral of children and families.
