Project Description:

The Polk County Health Department is requesting $172,140 for the design phase of anticipated new construction for a 11,702 square ft. clinic in Lake Wales.  This request is in response to a 1997 Department of Health Quality Improvement Site Visit, which cited this health department clinic as needing significant improvement with regards to providing a safe, clean environment for staff and clients.  Polk County Board of County Commissioners has committed to purchasing property for the new clinic that will be considered in-kind.  The existing clinic is comprised of three structures - a main building built over 25 years ago, a modular unit, which is over 15 years old, and a portable trailer, which has been leased for 9 years - a total of 4,288 square feet.  Several of the issues to address are:  Continual air-conditioning problems with mold growth issues; sever space limitations - the client laboratory is being housed in space originally designated as a janitors closet/mechanical room; inadequate number of examination rooms; lack of compliance with ADA; inadequate file storage capacity; and the clinic does not comply with the Department of Health policies and procedures on information security.

Attached documentation gives statistical and other information to support this request.

Services Provided/Benefit to State:

In fiscal year 96/97, the Lake Wales staff provided services to clients in 8,785 primary visits.  In addition, immunizations were administered to 2,614 additional clients.  These numbers have increased dramatically since that time.  The peak occupant load at the existing Lake Wales clinic is 50-65 people (15 staff members plus 35 - 50 clients) per day.  The benefits would be to improve the WIC/nutrition division; improve the Health Department's ability to provide health services in one of the higher growth areas of Polk County; and, increase client satisfaction with the services of the health department.

Measurable Outcome Anticipated:

Failure to implement funding for design of future construction will cause continued severe overcrowding, continued deterioration of an already obsolete facility and hinder the capability to service the clients that are in need of the facility.  If approved, the outcome would be assurance of continued compliance with ADA and Department of Health regulations along with better service to clients and improved working conditions for the staff.
