Project Description:

 

 

 

 

 

 

 

This continuation (and expansion) project will provide(s) post-crisis stabilization unit (CSU) out placement in transitional housing for chronic, mentally ill homeless persons.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

The continuation (and expansion) funding will allow the program to continue to provide appropriate supportive housing to homeless individuals who require stabilization services following out placement form [state-fundend] CSU's.  The program expansion enhances the post-discharge placement options for the state-funded CSU's and persons recently discharged and at greater risk of destabilizatin.  Homeless persons currently discharged from CSU's frequently de-stabilize without time and appropriate out placement or follow-up, often requre law enforcement intervention, and are recurrently readmitted into a CSU.  The availability of service intensive mental health treatment beds specifically designed to keep individuals stable and prepare them for permanent, supportive housing, will reduce the incidence of recidivism into the CSU system, producing cost savings for the state by reducing the use of CSU beds, reduce the length of stay at CSU's (by providing increased out placement options) and reduce law enforcement intervention time and costs for persons who de-stabilize following discharge from a CSU.  The expansion requested would fund an additional 35 beds (for a total of 50) to provide on site case management services, medication management, group and individual therapes, referrals to ancillary services (substance abuse, employment placement, etc.) and assistance in obtaining appropriate permanent housing with supportive services.
