Project Description:

 

 

 

 

 

 

 

Central Florida Health Care (CFHC)must renovate and acquire additional space for their existing pharmacy progam.  Funds will also be used to purchase new medical equipment.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

The working poor, uninsured, underinsured, publicly insured, frail, elderly, chronically ill, migrant/seasonal farmworkers and all others who lack access to comprehensive primary health care (medical, dental, pharmacy, education, etc) services from across the County receive services based on a sliding fee scale.  Our centers are extremely effective in teaching patients to end their reliance on emergency rooms and to use preventive care services--the true mission of a managed care system.  CFHC is one of 27 private, not-for-profit health care corporations spanning across Florida.  For over 27 years, the centers in Polk, Hardee and Highlands Counties have provided thousands of patients with medical and dental care. CFHC is a community-based health care corporation of, by and for the community governed by a 14 member community board.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

CFHC centers have a unique 27+ years track record of controlling costs, providing access to quality preventive and primary health care, retaining health professionals where they are needed most and working within our communities to develop long-range solutions to their health needs. Through reductions in hospital admissions and less frequent use of costly emergency room visits for routine services, Florida will continue to realize a considerable savings because the services provided by CFHC are less expensive and more comprehensive than the HMOs and private provider offices.
