This program seeks to address the needs of those homebound individuals who are unable to leave their home for medical care.
Services Provided/Benefit to State:
In the home, the team is able to conduct EKGs, blood tests, lab work and pulse oximetry in addition to basic physical exams.
Measurable Outcome Anticipated:
Care in the home will save thousands on emergency care and hospitalization.
