Project Description:

 

 

 

 

 

 

 

Neighborly Senior Services, Inc. (NSS) proposes to build a 5,250 square foot Adult Day Services/Health Center in Largo, Pinellas County.  The site is in the community of Ridgecrest, a targeted community development area.  The program will serve between 35 and 50 frail elderly clients per day who would otherwise be at risk for institutional care in a nursing home or hospital.  NSS provided services in this community for 25 years at a facility donated by a community church.  Unfortunately, in 1998, the church requested that NSS close the center as the church needed the space for congregational use.  As a temporary measure, the clients were transferred to the Clearwater Adult Day Services Center.  This has forced elderly Largo residents, many of whom have medical and cognitive conditions, to travel long distances to receive services that should exist in their own community and resulted in a waiting list of 31 clients for the Cleawater Center.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

Services to be provided at the Largo Adult Day Services/Health Center include:  medication management and nursing services under the supervision of a registered nurse; daily therapeutic activities designed to improve the social, cognitive and physical functioning of the at-risk elderly client; wellness groups; caregiver support groups; caregiver respite programs; counseling; and nutritional services such as nutritional risk assessments, daily meals and congregate dining.  Medical research has documented that caregivers receiving counseling services, including regular attendance at a support group, delay nursing home placement of Alzheimer Disease patients by 329 days when compared to caregivers not receiving such support.  The amount of funds requested would keep only 12 clients in a nursing home for one year, but will enable NSS to prevent or delay institutional placement for many elderly over many years.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

*Provide adult day care services to at lest 35 clients daily as measured by client census.

*Reduce the Clearwater Center wait list as measured by Center reports.

*Improve the quality of life of clients as measured by client outcome surveys.

*Improve the chances of frail elderly clients to remain in an independent living environment.
