Project Description:
To provide 125 hot onally balanced, home delivered meals to home bound elderly and other support services.
Services Provided/Benefit to State:
Improve nutrition of 125 elderly participants by assesing nutritional needs providing information and referral services.
Measurable Outcome Anticipated:
One hundred and twenty five (125) elderly will receive daily hot meals delivered five (5) days a week. (267 days a year)
