To provide 125 hot nutritionally balanced, home delivered meals to home-bound elderly and other provide other support services.
Services Provided/Benefit to State:
Improve nutrition of 125 elderly participants by assessing nutritional needs, providing information and referal services.
Measurable Outcome Anticipated:
125 elderly will recieve daily hot home delivered meals, five days a week, 26 days a year.
