|
The purpose of this project is to provide congregate and home delivered meals to the elderly residents of Miami-Dade County who, due to budgetary cutbacks in our current Older American's Act, will lose access to their meals services. This request would enable the continuation of uninterrupted services to frail, socially isolated, low-income elderly with hot nutritious meals and to reduce a portion of our waiting list clients. |

12,600 Home delivered meal units and 85,680 congregate meal units to 390 unduplicated clients. Self Care and the reduction and delay in nursing home placement and the prevention of acute episodes benefits the state by reducing overall costs of nursing home placement and costlier forms of home and comunity based long term care. |

This Self Care Elderly Meals Program will provide 95% compliance with the DOEA's three outcome measurements standards: 1.- Reduction of Nursing home placement, 2.- Prevention of acute episodes, 3.- Delay in Nursing Home placement. |
