Project Description:

 

 

 

 

 

 

 

Facility will house an accessible, centrally located, multi-use and elder friendly facility for senior activities; unite a full spectrum of services to accommodate the able bodied as well as the functionally impaired senior and increase service capacity to reach the growing number of seniors needing assisance to live independently.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

Comprehensive community based senior care and center based activities/Long term care resources are used in the most efficient and effective way.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

Decrease in high social isolation scores for increased number of seniors participating; double capacity for adult day care services.
