Project Description:

 

 

 

 

 

 

 

This Prevention and Family Preservation project will heighten awareness, promote family ownership to problem solving, offer a non-threatening connection, pre- and post-stabilization family support systems, and linking referral resources. Education and information classes are available to provide self-help tools and techniquues, as well as an accessible resource library.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

Hertiage is providing hands-on guidance and direction through monthly support group meetings, scheduled family support-parenting classes, neighborhood networks, and other collaborative partnerships. The state would benefit from simply implementing a similar model, that has measured outcomes of a peer-support linkage at the community base that promotes ownership to problem solving, and resolutions to the recurring and raising trends of child abuse and domestic violence issues.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

Out of the 200 families who participate in the Heritage Prevention and Family Preservation program: to encompass all program components, a minimum 75% will complete the program. In order to successfully complete the program participants may miss no more than one session of a class series.

1. a decrease in the number of at-risk families in District VII;

2. a decrease in the number of adoption disruptions;

3. an increase on the number of adoptive-foster placements;

4. an increase in the number of families seeking preventive services.
