Project Description:
To construct a community health center for Riverview/Gibsonton which will serve as a medical home to the residents of the area.
Services Provided/Benefit to State:
Primary health care including dental services to all patients regardless of their ability to pay. Patients must currently travel in excess of 20 miles for care, many with little or no access to transportation.
Measurable Outcome Anticipated:
Improved health outcomes, proper management of chronic illnesses such a hypertension, diabetes and heart disease. Timely immunizations, health screenings and other preventive measures to avoid more serious long-term illness in both the adult and child population.
