Project Description:

 

 

 

 

 

 

 

EWC is committed to addressing the economic health, social and human service needs of its neighbors.  EWC will address many of the unmet needs of citizens with the establishment of the "MediMinder" Program".  MediMinder will monitor current health conditions of senior citizens, lists current medications, schedule, type and number of medical appointments and other pertinent health data along with future health care appointments.  Seniors will monitor their MediMinders with a student volunteer, who will, at each meeting, ensure appropriate medical care and follow up has occured.  The program will be based on the EWC Campus in the Schell-Sweet Resource Center, which is shared with the University of Florida Health Clinic, in partnership with the City of Jax Community Services/Adult Services Division.  The Community Services and Gerontology Departments will establish a MediMinder Team to provide collaborative program management.  Program services will initially be offered three days per week, three hours per day for 75 seniors during the programs first year.  Access to the program will be provided by van service sponsored by the program.  Student volunteers will be solicited, registered, trained, assigned and monitored through the Community Services Department.  The Gerontology Department will provide training relative to adult care and services.  They will establish, provide and monitor all students training in the program area and provide feedback to students regarding effectiveness and productivity.

Enhanced medical awareness for seniors who otherwise may not be very attentive to their medical conditions for various reasons.  Participants will receive computer training, thus developing new skills, enhancing creativity via technologyy, along with small motor exercise for the hand.  Increased medical awareness will lead to a reduction in indigent care hospitilization cost.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

Improved medical care for the seniors. Improved psychological functioning, communication and psychomotor skills. Monitoring of medical care, policy and practice for a vulnerable group of individuals with different social and socioeconomic backgrounds.
