Transitional Living Program that provides housing and associated supportive services to homeless persons with severe mental illness and substance abuse.
Services Provided/Benefit to State:
Housing, mental health and substance abuse outpatient and day treatment, assessment, case management, residential support.

Maintenance of housing; improvement in functional level.
