Project Description:
Mental health services to include crisis screening, in-home counselors and related travel, advanced registered nurse practitioners for medication/ counseling,case managers for non-Medicaid elderly and near elderly, outpatient therapists, medication costs, education and awareness activities, and suicide prevention.
Measurable Outcome Anticipated:
Reduction in Baker Act and inpatient admissions, reduction in foster care length of stay, reduction of inpatient admissions, and improvement in mental health status of individuals who access services.
