Project Description:
Funding for the State Kidney Disease Program provides for continuation of our statewide educational and rehabilitative programs that ultimately prevent unnecessary hospitalizations and helps patients resume self-reliant, normal living.
Measurable Outcome Anticipated:
By providing intensive educational and rehabilitative services, the State Kidney Disease Program works to prevent the incidence of a large number of unnecessary and costly hospitalizations, prevent the occurrence of unnecessary transportation costs, and enable the patient, though suffering with a highly debilitating and chronic illness, to effectively manage their health care needs and resume productive lives.
