|
||
Bridges of America's 100 Bed Post-Release Transitional Housing Program provides therapeutic support, affordable housing,job placement assistance, and additional substance abuse treatment through referrals when needed. The program links services to offenders and inmates who are released from community and prison substance abuse programs. We also provide and link services to offenders who are currently on community supervision or has been released from prison or community supervision within ninety days and is accessed to need transitional housing services. The program is designed to meet the various needs of offenders and inmates experience when attempting to re-enter their communities. The case managers at each program location are responsible for the initial intake and assessment to determine what services are essential for the resident reintegration back into their communities. The overall mission of the Post-Release Transitional Housing Program is to assist post-released offenders by providing transitional housing and other such support services to assist them in maintaining a drug-free, independent living, crime free life style and make a successful reentry into the community. |

Measurable Outcome Anticipated: |

The Bridges of America's measurable outcome is based on three variables: (1) To be gainfully employed at termination of the program. (2) Suitable housing a termination. (3) The client and case manager have satisfactorily completed their agreed upon goals and objectives for successful discharge. Our anticipated outcome is predicted on the client having previous treatment for their substance abuse problem (at least 90 days residential care). The successful outcome goal is to maintain a 65-75% success rate for all eligible clients. The preliminary outcome report indicates that so far: (1) 46% of the clients that have left our transitional housing programs are classified as successful (met the three criterion for successful discharge). (2) 29% left the program as unsuccessful. (3) 4% left for administrative reasons and another. (4) 21% transferred to other programs i.e., residential mental health or substance abuse programs. |
