There is a continuum of direct services. They include Alzheimer's Programs, Corporation for National and Community Service, Day Care services, Home Care Services, Home and Community Based Services, Home Care for the Elderly, and Nutrition Programs.
Measurable Outcome Anticipated:
1. To provide information so that people can understand how to maintain/regain their maximum level of independence and self-sufficiency. 2. To delay premature institutionaliztion of elderly individuals. 3. Provide services to elderly individuals referred by case managers or outreach workers.
