Project Description:

 

 

 

 

 

 

 

The proposal would create a 20 bed adolescent residential substance abuse treatment facility. Insufficient adolescent based programming and juvenile jutice resources have been stretched to the limit in this community. Currently no facility provides treatment for adolescent abusing substances in this area. This facility would provide intensive moderate term substance abuse treatment for adolescents.Proposed length of stay would be 60-120 days. Core services for the adolescents in treatment would include: Assessment using ASAM,CSAI and others; health assessments; substance abuse cessation-focused individual, group, and family modalities; gender-specific services; multicultural issues groups; adolescent issues groups;academic assessment and classroom education; relapse prevention; discharge planning and follow-up; and customized interventions based on need.

 

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

CRC adheres to the performance outcome measures set by the state, following the data collection and submission requirements of the state. We are required to comply with and report data on established programmatic and client outcome objectives that provide point-in-time date that will measure program effectiveness. Additionally, CRC follows standards set by our accrediting organization, the Joint Commission on Accreditation of Healthcare Organizations, and utilizes the Customer Satisfaction Survey process.
