Project Description:
This project will support the Sun Coast Hospital Clinic, located on the site at a vocational school-based clinic (PTEC), in an underserved high risk area within Pinellas County. Screening, assessment, education, treatmnet, counseling and referrals will be provided in both primary care and outreach setting for clients of all ages. This multi-disciplinary approach will provide the following services: Assessing health needs of the community, including students at the vocational school; health and lifestyle screening; couseling at-risk behavior; prevention of teen pregnancy/sexually transmitted diseases, family planning, and HIV counseling. Interagency referrals will promote client access to Pinellas County Health Department including Healthy Start, WIC, Dental, Teen Pregnancy Prevention, and Breast and Cervical Cancer Screening Program.
Measurable Outcome Anticipated:
Services provided under this contract would be those specifically designed to serve High Point/PTEC community. The provider shall achieve an 85% or better rating on a client satisfaction survey/periodic evaluation administered by the provider. A quarterly demographic report will be completed to further ensure that the desired performace outcomes are met.
