Project Description:

 

 

 

 

 

 

 

The continued implementation of an aftercare residential program for Dual Diagnosed (Mental Illness and Substance Abuse) individuals released from Correctional facilities.  Funds now expended on rearrest and reimprisonment for these individuals can be reduced and this will assist in reduction of criminal acts that are drug related and its consequences.

 

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

70% of severely mentally ill partipants will start to receive health treatment voluntarily within six months; 30% of substance abuse participants will successfully complete substance abuse treatment within one year; 40% of participants without income/entitlement will apply for SSI within 60 days; 65% of participants without life skills will voluntary enroll in day treatment and start day treatment services within 60 days; 80% of participants will maintain permanent residence for at least one year; 40% of participants will enroll in employability skills training within 90 days of admission.
