Project Description:

 

 

 

 

 

 

 

Senior Friendship Centers, Inc., and Jewish Family and Children's Services are working together to address the needs of isolated, at risk older adults. In a program called SCOUTS vulnerable seniors are identified and given the assistance to meet their immediate needs. Some of the increasingly serious and interrelated problems include: elders living in unfit environments that include no food, unsanitary conditions, unmet physical and emotional needs; individuals in their eighties and nineties who are blind, wheelchair bound, physically disabled, confused, disoriented, have no running water, no electricity and no resources available to correct these indignities ; unmet medical needs due to lack of financial resources; social isolation leading to depression; disheveled appearance, bedridden incontinence; fear and paranoia of institutionalization.

 

The SCOUT program addresses these needs and engages and educates the public in identifying isolated and vulnerable frail elders; SCOUT provides multi-disciplined services and mobilizes community resources to address immediate needs; works with individuals, families, community and faith-based organizations to develop longer term solutions; helps frail elders age in place with dignity while preventing costly premature institutionalization.

 

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

With additional funds another two teams of (2) persons each team can be put in place in mid and north Sarasota County.This would allow 100 consumers per team to be seen; 90 new assessments per team; 25 training classes per team; 300 community members trained to identify these vulnerable adults; 20 consumers per team will receive mental health assistance.
