||
This contract will support the Sun Coast Hospital Clinic, located on site at a vocational school-based clinic (PTEC), in an underserved high risk area within Pinellas County. Screening, assessment, education, treatment, counseling and referrals will be provided in both primary care and outreach setting for clients of all ages. This multi-disciplinary approach will provide the following services: assessing health needs of the community, including students at the vocational school; health & lifestyle screenings; counseling at-risk behavior; prevention of teen pregnancy/sexually transmitted diseases, family planning & HIV counseling; community education with emphasis on chronic diseases; providng a medical home for target population, including prenatal and pediatric care. Interagency referrals will promote client access to PinCHD services including Healthy Start, WIC, dental, teen pregnancy prevention and breast and cervical cancer screening program. |

Services provided under this contract would be those specifically designed to serve the High Point/PTEC community. The provider shall achieve an 85% or better rating on a client satisfaction survey/periodic evaluation administrered by the provider. The provider will ensure that all staff under this contract receives appropriate and relevent training on a regular basis. The client satisfaction surveys or periodic evaluations will be conducted at least quarterly and involve a sampling of at least 10% of the records of the persons who have received services during the contract year. The provider will be monitored both programmatically and administratively a minimum of once per contract year. A quarterly demographic report will be completed to further ensure that the desired performance outcomes are met. |
