Provide hot evening and weekend meals to elderly residents of Miami-Dade County who are assessed as "high risk" or in danger of malnutrition.
Measurable Outcome Anticipated:
Will provide compliance with the Department of Elder Affair's outcome standards: (1) reduction and delay in nursing home placement, (2) prevention of acute episodes and (3) increase in community based programs.
