Northwest Quadran Community Health Centers' overll mission is to provide available and promptly accessible primar health care services to the medically underserved community of Jacksonville, nd to assure the continuit of health care servcies by coordinating program with other provides of health care services to the low income community.  We propose Procurement of a new facility infrastructure for administrative building and a new medical facilty for the provision of of comprehensive primary care for pediatric, adolescent and adult care.  This facilty will house (1) Indigent Hospital (2) Health Department (3) Are Health Planning Council (4) Mental Health Provider (5) Area substance abuse services (6) Dental provider.  All agencies will participate at least 25% through referral and possibly in-kind services.

 

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

Improved health status for children, elders, minorities and immigrants;  Improved health care services including, but not limited to, reduction of chronic diseases.  Prevention and treatment for substance abuse and mental health clients. Improved therapeutic services to children.  Improved prenatal and infant care; Reduction in communicable diseses; Improved access to community based treatment alternatives; Improved school readiness  Improved health nd social service infrastructure.
