Project Description:
Will provide guardianship services allowing access to home and community based services to the most frail, incapacitated elder population, thus allowing them to reside in the community longer rather than being institutionalized, also giving them access to proper and cost-effective medical services.

Avoidance of premature institutionalization by providing quality decision-making capabilities.
