House an accessible,centrally located multi-use and elder friendly facility providing an ease of access to the continum of long term care, expand the operations base for comprehensive home and community based services and expand partnerships with acute care facilities such as hospitals, nursing homes and assisted living facilities. Unite a full spectrum of services to accommodate the able bodied as well as the functionally imparied elderly. Increase service capacity to provide home and community based services to reach the growing number of seniors needing assistance to live independently. |
