Project Description:

 

 

 

 

 

 

 

Funding for maintaining 8 dementia specific day service programs in Palm Beach and Martin counties.  These dollars would fund daycares that are specifically designed to care for Alzheimer's Disease Initiative (ADI) program administered by the Florida Dept. of Elder Affairs.  The funds will be used for those patients who cannot afford daycare expenses and for care management duties in accordance with the ADI guidelines.  The current day service sites, which are all community based, are located in Boca Raton, Delray Beach, Lake Worth, West Palm Beach, Pahokee, Palm Beach Gardens, Stuart and North Stuart.  The $800,000 will directly support expenses, including salaries for direct care staff.  Volunteers will be used to uplift and enhance the quality of care and to keep cost at an affordable and accessible level for participants.  The cost for maintenance of the daycare space will be shared with the donors.  State funds will not be used for rent, construction or administrative costs.

 

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

650 unduplicated patients will be served and receive dementia specific care through the eight daycare programs.  All participants will be registered with the National Alzheimer's Association Safe Return Bracelet Program, receive daily personal care (showers and hand bathing), medically monitored daily by nurses who specialize in dementia specific behavior and health care, and will be involved with therapeutic activities provided by professionally trained staff and volunteers.  As a result, daycare delays premature institutionalization by two or four years.  In addition, 700 unduplicated family members will receive education on the disease process, a quarterly Alzheimer's educational magazine, care management through ACCA's Family Consultant Program, access to weekly support group meetings at each site and information on appropriate and adequate resources through ACCA's information and referral services.  ACCA estimates there are 62,000 people suffering with Alzheimer's disease or related disorders, living in Palm Beach and Martin Counties - the largest concentration of this fragile population in the country.  Because of these daycare sites, patients and the community will have access to options that are human and dignified, which is the hallmark service for Palm Beach and Martin Counties.
