Project Description:
Construction of a 30,000 sq. ft. facility specifically designed to serve 100 adult day care participants, 200 nutrition participants and comprehensive commmunity based services including day health care & monitoring, personal care, Alzheimer's and dementia respite care.
Measurable Outcome Anticipated:
Provision of home & community based services to those most in need, enabling elders to remain in the least restrictive environment with intact families and to delay institutionalization, the most expensive level of care.
