Project Description: |
This program would provide care for financially limited people challenged with Parkinson's Disease and/or related movement disorders, including people with spastic muscle tone, often seen following a stroke or other neurological injury. Specifically these people could be identified by caregiver, neighbor, or medical peovider as being unsafe alone in their home during the day, or requiring more assistance than their spouse or caregiver can provide on a daily basis, even if their limitation to be able to provide care would be "burn out" physically, mentally or both. The people challenged with the physical limitations would be involved in a supervised Adult Day Program, as well as receive Rehabilitation Services to maximize their ability to be independent in their home environment. By receiving supervised care and rehabilitation services to maximize independence, the caregiver would be given the support needed to help them in keeping their loved one at home. Hours of operation would be Monday-Wednesday-Friday 8:00am to 4:00pm. (Use of the requested funding would be calculated for the Adult Day Program at $6.50/hr. as is the current rate. Rehabilitation service provision would be calculated at $50.00/hr. For each recipient proving financial hardship, Adult Day Program would be limited to 1-2 times per week, and rehabilitation services would be limited to 1-2 times per week, taking care to provide services to as many people as possible - statistical use of the funds will be maintained internally and reported as requested.) |

We will track these program participants associated with the following: 1. Their initial level of functional independence; 2. Improvements in their activities of daily living and safety awareness; 3. The caregiver's initial ability to care for their loved one; 4. The development of the caregiver's increased skills and tolerance to provide assistance to their loved one; 5. Number of clients that can continue to reside in their homes. |
