Project Description: |
This proposal requests the continuation of $180,000 appropriated in 2002-2003 and the expansion of an additional $120,000 in 2003-2004 in funding for Outreach to the Elderly for Medical Compliance Substance Abuse and Mental Health. The misuse or abuse of alcohol and precription drugs among America's elderly population is one of the fastest growing health problems today. River Region Human Services proposes to address the above referenced issues through intensive outreach to the elderly in the institution of these programs: (1) Education about the proper use of prescribed medications, (2) Follow-up with the local hospitals regarding hip fractures, falls and accidents, (3) Community training relative to the complications surrounding medicating the elderly and the rising use of alcohol and other drugs. The goal of the project is to achieve medical compliance and prevent complications that involve citizens in Duval, Clay, Baker, Nassau and St. Johns Counties. The impact willresult in the reduction of emergency room visits, decrease in the use of contraindicated medications, informed senior citizens who can negotiate medications and intensive medication monitoring systems that are client driven and can be replicated throughout the state. |
