Project Description:

 

 

 

 

 

 

 

This project is for Senior Center Construction and renovation matching grant.  Provide services to low income seniors at risk of state Medicaid institutionalization.  City will provide a continuim of community based services at the Center.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

Statewide benefit in maintaining functionality, self sufficiency, and optimum independence of low income seniors.  Hot meal, socialization, recreational activities provided to reinforce self worth.  State benefits by providing community service instead of state Medicaid nursing home placement.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

The outcomes will be tested and measured against the general elderly population.  The outcome desired is to maintain elderly in community setting by providing services.
