PROGRAM DESCRIPTION

 

Bridge the Gap- Empowerment Center, Inc. is a minority community based organization.  Order My Steps AIDS Ministry is the program providing prevention to address the high priority HIV prevention needs in African American and Latino populations.  Order My Steps provides services in the areas of case management, education, prevention counseling, testing, outreach, formal referral services, transitional housing, transportation and network through collaborative efforts with other community based providers.

 

Our activities include workshops, seminars, health fairs, and HIV/AIDS classes and courses.  This component and all components will be strategically provided at various sites within the community through collaborative efforts with other service providers.

 

The organization is involved with prevention by structuring classes, seminars, distributing condoms, and information about HIV/AIDS to the non-infected, the infected, and affected community.  Our goal is to have a direct effect on the target population.  As a certified testing site, one of our methods is to test for HIV/AIDS.  This will in turn provide a link to STDs, TB, and substance abuse providers.

 

The organization is constantly involved in outreach.   The outreach includes door-to-door involvement by our organization, flyers being distributed, seminars to the community, and media announcements.  Our outreach includes materials one can employ to avoid infection.

Service provided HIV / AIDS prevention, education, outreach, counseling and testing programs.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

1.         Two Case Managers ? measurable outcome will be determined by the number of clients that case mangers are able to access (35 per case manager is the state maximum)

2.         Workshops and Seminars ? The number of persons in attendance

3.         Education and prevention Measurable outcomes will be determined Pre and Post Tests ( a universal standard for testing knowledge in all fields) and the number of condoms distributed

     4.   Transitional Housing ? will be determined by the number of clients serviced
