Project Description:

 

 

 

 

 

 

 

The Florida Community Health Protection Act of 1999 established a pilot project in Broward County for the communities surrounding the Miles Health Center in Fort Lauderdale.  The purpose of the project, for which funding is sought, is to promote disease prevention and health promotion among the low-income persons living in that area and to eliminate disease disparity so that the low-income persons can participate in economic, environmental and health revitalization of the area.

 

 

 

 

 

 

 

 

 

 

Services Provided/Benefit to State:

 

 

 

 

 

Services provided to the state include: 1) demonstration of techniques and approaches to ensure community-based, culturally sensitive  health care for disease prevention and health promotion for low-income people; 2) promotion of partnerships between existing health care providers and community-based organizations to promote health and prevent disease for low-income people; and 3) development of educational and outreach programs for community-based organizations and health care providers that increase awareness of health care needs for low-income persons in the area and opportunities to meet those needs

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

Outcomes anticipated include the implementation of community-based, cultural sensitive health care services which prevent disease and promote health in low-income communities; model approaches for achieving disease prevention and health promotion in minority communities; and partnerships between government, community-based organizations, and the private sector which promote economic, health and environmental revitalization of low-income communities
