Project Description:
House an accessible centrally located multi-use and elder friendly facility for senior activities. Unite a full spectrum of services to accommodate the able-bodied as well as the functionally impaired senior and increase service capacity to reachthe growing number of seniors needing assistance to live independently.
Services Provided/Benefit to State:
Comprehensive community based senior care and center based activities/long term care resources are used in the most efficient and effective way.
Measurable Outcome Anticipated:
Decrease in high social isolation scores for increased number of seniors participating: Double capacity for adult day care services.
