Project Description:
Nurses provide home visits for families with vulnerable children & deliver specific service interventions: HIV/testing and peer counseling during non-business hours. Environmental Health Services including rodent control and sanitary nuisance reduction.
Services Provided/Benefit to State:
Care in a non-traditional setting improving outcomes and improving overall health. This reduces the number of emergency care visits needed.
Measurable Outcome Anticipated:
Improved infant mortality; improve children's health; reduce the HIV infection rate...to address racial and ethnic disparities in health outcomes; decrease TB & STD rates.
