Project Description:

 

 

 

 

 

 

 

We are continuing an existing program that provides Residential Level 2 housing to persons with severe and persistent mental illness.  Non-recurring funds were provided during the 2001 Legislative Session to expand the program.  That expansion has now moved forward.

 

Participants live in supervised housing units within a community apartment complex.  This set-up provides participants with opportunities for interaction with successful program graduates and other independent community residents.

 

Services include assessment, case management, nursing services, adult day treatment, structured activities, crisis intervention, milieu therapy, vocational services, and service integration.

 

Staff work together with the providers psychiatric team, case management, and mental health and rehabilitation day programs to develop and/or augment basic independent living skills which assist chronically mentally ill persons in living successfully within their communities.  The facility is staffed 24 hours per day/seven days a week.  Case managers and psychiatrists also serve clients.
