Project Description:

 

 

 

 

 

 

 

Free primary health care with referral to cooperating specialists and free prescription medication and hospitalization to people who have no health insurance and incomes no higher than 150% of the Florida Poverty Guidelines.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

Specifically, the Clinic proposes to provide the following services during the program year: Primary Diabetes/Hypertension screening expanded to 400 adult patients; Primary Diabetes/Hypertension screening expanded to 750 new pediatric patients. Secondary Diabetes/Hypertension screening (hematology, i.e. blood glucose & lipids) provided to 400 new adult patients; Secondary Diabetes/Hypertension screening provided to 300 pediatrict patients; Retinopathy camera will be used to screen 20 new patients for vascular pathology (early warning sign of diabetes and hypertension); Culturally sensitive nutrition classes offered twice monthly and promoted through the Migrant Education Program; Culturally sensitive hygiene classes offered twice monthly and promoted throught the Migrant Education Program; Food/produce supplements will be provided to 1,500 at-risk patients; Recruitment and training of community representatives (volunteers) to assist with screening and patient care.
