Project Description:
This project will serve individuals that experience severe mental illness and have been identified by the Miami-dade County Courts as in need of diversion to community-based treatment and rehabilitation. The goal of the project is to assist these individuals to become engaged in treatment, rehabilitation and support services that are essential to their long term sucessful adaption to community living. This will be accomplished by providing comprehensive psychosocial rehabilitation services, i.e., housing, case management, psychosocial rehabilitation, outpatient treatment and community employment.
Measurable Outcome Anticipated:
Specific outcomes will include decreased incareration and psychiatric hospitalization recidivism and increased functional capacity and community tenure.
