Project Description:
Health Choice Network is requesting support from the state of Florida for the continued operation and expansion of the Jesie Trice Cancer Prevention Project/Healthy Body Healthy Soul prgram. The Healthy Body, Healthy Soul program represents a sound approach to addresing health disparties affecting minority communities. Healthy Body, Healthy Soul uses collaboration among community health centers across the state faith based and other partners to extend outreach efforts to populations that are traditionally hardest to reach to provide education and change health behaviors effectively, and to provide a higher level of coordinaion of care than previously existed. Churches and other community partners enjoying proven credibility with families are engagind in efforts to educate their communites and change health behavior in a private way.Healthy Body, Healthy Soul is a statewide project that is a locally driven and locally controlled effort, based n solid partnerships among private, not for profit organizations with track records of serving predominately Black and Hispanic communities that span several decades. Instructures are in place at the Network level and in communities targated t plan, operate and evaluate Healthy Body Healthy Soul initiatives. Full implementation of the Project is underway in the first four target communities, with a focus on lung cancer prevention education and early detection of lung cancer- community screening is occuring, smoking cessation assistance is being provided, smoking preventions activities are being conducted, and patients at high risk for lung cancer are being referred for clinical screening (CT scans) to detect the diease. Planning is under way in a fifth community, with focus on prostate cancer. Sufficient ground worf has been completed and relationships among partners solidified to enable sustenance, strengthening and efficient replication of Healthy Body, Healthy Soul projects to maximize the Programs imact in FLoridas low income minority communities.
