Project Description:

 

 

 

 

 

 

 

The Focused Outreach and Intervention Program(Focused Outreach) is a collaborative effort by five local mental health agencies that provide timely and comprehensive mental health services to adults who have had difficulty angaging in treatment or have been recently released from jail or prision. The partners include; Directions for Mental Health, Inc.; Personal Enrichment through Mental Health Serevices, Inc.; Gulf Coast Community Care; The Harbor Behavioral Healthcare Institute, Inc.; and Suncoast Center for Community Mental Health, Inc, as the lead agency. The program targets individuals who have made multipe visits to local emergency rooms, have had frequent admissions to patient psychiactric unints or have utilized the services of the Pinellas County Department of Social Serives with some indication the problem may be related to mental illness. The program assists clients by linking them directly with psychiactric services and teaching them how to access and follow-up with traditional outpatient programs.

 

 

 

 

 

 

 

 

 

 

Measurable Outcome Anticipated:

 

 

 

 

 

The effectiveness of the program include the standard measure for mental health programs used throughout the state - improvement in functionong and improvement in days in the community(i.e., not in jail or hospital). In addition, an ouitcome of a decrase in repeated use of the referring public systems is proposed.
