| Hope Hospice is requesting funding to establish a hospice based PACE program in Southwest Florida. The PACE program allows seniors of our state with chronic care needs to be served in the community when possible. For an all-inclusive capitation rate, the medical cost reduction program provides eligible seniors with adult day care centers, offering nursing, therapies, meals, nutrition, social work and personal care. Services offered include: primary medical care, prescription drugs, home health care, social services, and respite care. | |||||||||
