| Focused Outreach targets individuals in the Criminal Justice System who are diagnosed with severe and persistent mental illness. This program links with them prior to release to accomplish a two-fold goal: to increase the likelihood that they will follow up with community-based services upon their release, and to decrease the likelihood that they will be re-incarcerated in the future. This porposal supports and enhances the existing program, with the inclusion/expansion of a receiving center, operated by local community mental health agencies. Services include: immediate evaluation and assessment; psychosocial assessment; individual & family therapy; psychiatric evaluation; medication therapy; assistance in locating housing; assistance in meeting parole or probation requirements; advocacy in court proceedings; care coordination; group therapy. Outreach is conducted in many locations (including jails, hospitals, homeless shelters, and halfway houses). | |||||||||
