494.0066  Branch offices.--

(1)  A branch office license is required for each branch office maintained by a licensee under ss. 494.006-494.0077.

(2)  The department shall issue a branch office license upon receipt of a completed application form as prescribed by rule by the department and an initial nonrefundable branch office license fee of $325. The branch office application must include the name and license number of the licensee under ss. 494.006-494.0077, the name of the licensee's employee in charge of the branch office, and the address of the branch office. The branch office license shall be issued in the name of the licensee under ss. 494.006-494.0077 and must be renewed in conjunction with the license renewal.
